GROUP COMMUNITY SESSION CONSENT AND CONFIDENTIALITY AGREEMENT

RH Care, P.A. and its affiliated covered entities, (hereafter collectively "RH Care," "we," or "our") provides, through the Revero Inc. technology platform (the "Revero Platform"), its patients with the ability to engage in virtual group community sessions (each a "Community Session"), which may be led by a health coach or by a licensed health care provider in a group setting. The Community Sessions will be used by RH Care as a mechanism to onboard and welcome individuals to the Revero Platform and to provide Group Members (a defined below) with a forum to interact with other individuals on the Revero Platform, where individuals may share information that is personal including but not limited to health information, and to receive information pertaining to the services of RH Care. This Group Community Session Consent and Confidentiality Agreement (the "Agreement") describes how RH Care expects its members participating in Community Sessions to conduct themselves and what to expect. By consenting to this Agreement, you understand and agree to the terms and conditions of this Agreement and consent to participate in Community Sessions.

Consent to Community Sessions:

RH Care strives to maintain a positive and open environment during its Community Sessions. The ability to engage and interact with other individuals in a Community Session (collectively "Group Members") in a safe and healthy manner is RH Care's primary goal.

RH Care appreciates that participating in Community Sessions may lead you to have reactions, both positive and negative, to your fellow Group Members or your Community Session facilitator. If at any time, you feel that a Community Session is not appropriate for you or you have an issue with another Group Member, please contact the group facilitator or RH Care to discuss potential options.

Community Sessions are not designed for emergency situations. If you are experiencing a medical emergency, please dial 9-1-1 immediately.

Group Member Conduct:

  • RH Care believes that discussion within a Community Session can be a valuable tool in providing our members care. We strive to create a safe space for Group Members and acknowledge that sometimes Group Members may share personal and sensitive information in a Community Session. As such, you agree to:
  • Keep Community Session information confidential, and to not repeat or share any information about another Group Member, including their identity or any information shared in a Community Session, in any manner outside of the Community Session.
  • Respect the privacy of your fellow Group Members. We encourage free and open communication within the Community Session and strive to create an environment that promotes safe and effective communication. We also understand that Group Members may communicate with one another outside of a Community Session. We encourage each Group Member to only communicate with other Group Members if they are comfortable doing so and you should never feel obligated to talk to a fellow Group Member if you are uncomfortable or do not wish to share information with another Group Member.
  • Show respect to other Group Members at all times. A Group Member will not harass or bully another Group Member, in any manner, before, during, or after a Community Session, including on social media, phone, text, email, or any other manner whatsoever.
  • Never record, either through a visual or audio medium, any Community Session.
  • Understand that from time to time, as a result of technology failures or the misinterpretation of audio and visual cues that may occur as a result of conducting virtual Community Sessions, you should be prepared to ask for clarification or provide clarification related to comments or statements made during a Community Session.

Group Session Privacy:

RH Care values patient privacy and Group Members should be aware that anything said in a Community Session will be treated by RH Care as if it is medical information. However, Group Member acknowledges that:

  • All Group Members agree to keep group conversations and information about other Group Members confidential. RH Care takes measures to ensure that your private information remains confidential. However, despite these safeguards, a fellow Group Member may share your confidential information. We encourage our Group Members to be thoughtful about what they share in a Community Session and consider whether the information that will be shared is appropriate to be shared in a Community Session or better to be shared in a more private setting such as a one-on-one appointment with a health coach or licensed clinician.
  • RH Care strives to create a safe environment where Group Members can share and learn from one another. However, RH Care cannot control everything said in the Community Session that might be triggering or offensive to a Group Member. We value your feedback, and we ask that you communicate with RH Care if you would like to switch groups or if you have any other concerns.

There are exceptions to these confidentiality requirements. In some situations, your Community Session facilitator may be required by law to disclose information without your permission. For example, if the Community Session facilitator believes that you are a danger to yourself or others or if RH Care receives a subpoena or court order requiring us to disclose information, RH Care may be required to share your information.

By clicking the check-box associated with this Agreement you are acknowledging you have read, understand, and agree to this Agreement. Failure to abide by the terms and conditions of this Agreement could result in a suspension or termination of the ability to participate in a Community Session as a Group Member.
